Care Manager- Patchogue NY

Job Description

Care Managers help individuals achieve improved healthcare outcomes. Our staff provide field-based support that assists individuals with medical and behavioral health conditions through the following services:

-Assessment
-Care Planning
-Connections to Primary and Specialty Care Providers
-Service Coordination with Treatment Providers
-Client Education on Diagnoses, Medications and Treatments
-Linkages to Community-based Resources/Entitlements

*Must own a vehicle and possess a valid drivers license*

Requirements:

*Bachelor’s Degree in human services, social work, or related field

OR

*Associate’s degree with 1 year of case management experience

OR

*High School Diploma with 2 years of case management experience

Salary: $50,000/year for Bachelor’s degree; $45,000/year for Associate’s degree/High School Diploma

Schedule: Monday through Friday in person; hybrid schedule after successfully working 90 days based on program need
