Concern Housing is seeking a Case Manager to provide direct support services that help clients maintain stable housing and work toward independence. The role includes coordinating service plans, connecting clients to community resources such as clinical care, vocational programs, and benefits, and providing ongoing counseling and support. Responsibilities also include medication monitoring, housing related assistance, crisis intervention, site visits, maintaining documentation, and ensuring compliance with program requirements while serving as a consistent advocate for clients.
Days/Hours: Monday – Friday; 9am – 5pm
