Medication Manager

Job Description

Concern Housing is seeking a Medication Manager to support clients with safe and consistent medication management. This role includes monitoring medication adherence, providing education on prescribed regimens, coordinating with providers and pharmacies, and ensuring clients have the necessary supplies to properly store and manage medications. The Medication Manager helps clients build skills to manage medications independently, maintains accurate documentation, and collaborates with the care team to support overall stability and wellness.

Days/Hours: Monday – Friday; 9am – 5pm
