Position: Children’s Care Management Team Coordinator
Compensation: $60,000-$64,000 (based on experience and credentials)
Location: Ronkonkoma, NY and travel throughout Suffolk County
Hours: Monday-Friday 8:30am-4:30pm; occasional weekend and evening availability required.
Start Date: Immediate

Position Description:
We are looking for a highly skilled individual who can assume the role of Children’s Care Management Team Coordinator. Responsibilities combine duties of a professional direct service provider with supervisory and administrative tasks to further our mission.
New York State continues to undergo massive changes in a move towards integrating mental and physical health care, substance abuse and social service needs. The goal of the Children’s Care Management program is to work with children and their families, who have an array of behavioral health and medical conditions, to assist them in accessing medical, behavioral and social services in their communities so that unnecessary hospitalizations and emergency room visits can be avoided.
Key responsibilities will include:
• Supervising a Team of Children’s Care Managers and monitoring their progress and compliance with Core Competencies in Health Home Care Management
• Maintaining a small caseload and providing core Health Home Care Management services to assigned children
• Providing oversight and monitoring productivity of Children’s Care Managers
• Conduct ongoing risk assessment for all children on team
• Analyzing and running reports in electronic health records and other IT platforms and utilizing such to manage the team
• Auditing/monitoring records to ensure quality and timely completion of documentation
• Providing group and individual supervision to Children’s Care Managers on team
• Training and monitoring new staff
• Participating in staff meetings, Leadership meetings and other administrative meetings/trainings as required
• Performing duties with designated amount of time in field and office setting
• Working collaboratively with Program Leadership and Program Director and Senior Director of Care Management Services
• Monitoring staff hours, timesheets, mileage reimbursement requests

Qualifications:
• Two years’ experience working with children in the target population utilizing a person-centered, trauma-informed approach
• Strong leadership and supervisory skills including familiarity with an electronic health record
• Strong engagement skills and the ability to help the team improve and refine their own skills around engagement and client services
• Basic knowledge of the public health/mental health system, public entitlement and social service programs
• Excellent written and verbal communication skills
• Must be a reliable self-starter with the ability to work both independently and as part of a team
• Ability to work across diverse populations
• Bi-lingual (English/Spanish) candidates are strongly encouraged to apply. .

Education:
• A Master’s degree in a human service/health discipline; NYS licensure (or working toward) preferred; and two years administrative/supervisory experience.
• Current or previous certification in completing Child and Adolescent Needs and Strengths Comprehensive Multisystem Assessments (CANS) a plus
