Job Description

Direct Support Professionals support adults with intellectual and developmental disabilities in our Community Living Services program. You’ll help people build independence, stay safe, and live meaningful lives at home and in the community.

What You’ll Do
• Support daily routines such as personal care, hygiene, meals, and household chores
• Encourage independence by providing only the support each person needs
• Follow treatment plans and use adaptive or safety equipment as required
• Assist with cooking, portion control, and meal preparation
• Administer medications according to AMAP guidelines
• Complete documentation including case notes, goal tracking, and communication logs
• Provide transportation to appointments, shopping, and community outings
• Participate in team meetings and communicate progress with staff
• Help maintain a clean, safe, and welcoming home environment
• Mentor new staff when needed
• Perform additional duties as assigned

Requirements
• High School Diploma or equivalent preferred
• Valid NYS driver’s license and ability to maintain qualified driver status
• Ability to follow written and verbal instructions
• Must pass a fingerprint/criminal background check
• Ability to lift, transfer, and physically support individuals as needed
• POM training required (provided by the agency)
